Abstract

Anemia kehamilan di Indonesia berkontribusi 50–70% terhadap kematian dengan prevalensi 24,5%. Walaupun penanggulangan anemia dengan pemberian 90 tablet besi selama hamil telah dilakukan. Tujuan penelitian untuk mengetahui perbedaan kadar Hb, kualitas pelayanan antenatal dan budaya masyarakat, untuk mengetahui hubungan pelayanan antenatal dan budaya masyarakat dengan perubahan kadar Hb. 

Penelitian ini merupakan studi analitik komparatif dengan rancangan cross sectional pada ibu hamil masing-masing berjumlah 20 orang di puskesmas Gandus (cakupan rendah) dan puskesmas Swakelola Pembina Palembang (cakupan tinggi) diambil dengan acak sederhana. Analisis dilakukan pada kelompok anemia dan tidak anemia di puskesmas cakupan rendah dan kelompok anemia dan tidak anemia di puskesmas cakupan tinggi. Pengujian hipotesis menggunakan T-test unpaired, Ttest paired perubahan kadar Hb, Mann-whitney U dan Kruskal-wallis untuk mengetahui perbedaan kadar Hb pada kelompok I (p=0,007) dan III (p=0,011), perbandingan perubahan kadar Hb rerata tertinggi kelompok I (0,46), kelompok II, III dan IV berturut
turut -0,09, 0,43 dan 0,11. korelasi Rank-spearman dan Wilcoxon untuk uji hubungan pelayanan antenatal menunjukkan perbedaan bermakna (p=0,006. Hasil analisis skor budaya masyarakat menunjukkan perbedaan bermakna (p=,012), skor median
71 vs. 59,5. Selanjutnya korelasi perubahan kadar Hb setelah pemberian tablet besi dengan variabel diteliti hanya budaya mayarakat di puskesmas cakupan rendah r=0,0511, p=0,021.
Simpulan terdapat perbedaan kadar Hb ibu hamil sebelum dan sesudah pemberian tablet besi di kedua kelompok puskesmas. Kualitas pelayanan antenatal di puskesmas cakupan tinggi lebih baik dari puskesmas cakupan rendah. Budaya  masyarakat berpengaruh pada pemberian tablet besi. Perubahan kadar Hb tidak berhubungan dengan kualitas pelayanan antenatal. Budaya masyarakat sangat berpengaruh pada asupan tablet besi hanya di puskesmas cakupan rendah.
